		STATCARE Information Systems Forms

		Air Methods Corporation is committed to protecting its employees, resources, patients and partners from damaging or illegal actions by individuals, either knowingly or unknowingly. Computer resources are one of our valuable resources and shall be protected from theft, misuse, destruction or disclosure under applicable law, including Protected Health Information (PHI) covered under the Health Insurance Portability and Accountability Act (HIPAA) of 1996. The following forms will be used to add, restrict, or remove users from the STATCARE Information Systems. All forms can be printed and the completed form can be mailed or faxed to STATCARE. Network User Authorization: Used to create an Information Systems network account Network User Agreement (included in Network Authorization Form) Edit Network User: Used to edit a current user's name, paging level, payroll configuration, or access level Restrict/Remove Network User: Used to temporarily restrict a user's access, reinstate user's access, or delete a user For additional information please contact John Hultgren, Regional Communications Manager, at 502.479.9103.
